What Are All-on-4 Dental Implants?
All-on-4 is a full-arch restoration technique that uses four strategically placed dental implants to support a complete upper or lower set of replacement teeth.
Unlike traditional implant-supported dentures that may require six to eight implants per arch, this approach maximizes available bone and provides stable support with fewer implants.
The prosthesis attaches securely to the implants, creating a permanent solution that functions and feels like natural teeth.
How Does The All-on-4 Technique Work?
The procedure involves placing four dental implants at specific angles in your jawbone. The two front implants are positioned vertically, while the two back implants are angled at approximately 45 degrees.
This strategic positioning allows us to avoid areas where bone loss is most common and take advantage of the strongest bone in your jaw. The angled placement also provides better support and distribution of biting forces across the prosthesis.
After the implants integrate with your bone, we attach a custom-designed prosthetic arch that contains a full set of teeth. This prosthesis is fixed in place, meaning you cannot remove it yourself, providing the stability and function you've been missing.
Who Is A Good Candidate For All-on-4?
This treatment works well for patients who have lost most or all of their teeth in one or both arches.
You may be a candidate if you currently wear dentures and want a more stable solution, if you have multiple failing teeth that need extraction, or if you've been told you don't have enough bone for traditional dental implants.
During your consultation, we evaluate your bone density, jaw structure, and overall oral health.
Many patients who were previously told they needed bone grafting find that All-on-4 provides a viable alternative because the angled implant placement utilizes existing bone more effectively.
What Can I Expect During The Procedure?
We perform All-on-4 placement using local anesthesia, though sedation options are available for patients who prefer additional comfort. If you have remaining teeth in the arch being treated, we extract them during the same appointment.
Next, we place the four implants into predetermined positions in your jawbone.
In many cases, we can attach a temporary prosthesis the same day, allowing you to leave our office with functional teeth. This immediate restoration approach means you won't spend time without teeth during the healing process.
The procedure typically takes several hours, depending on whether extractions are needed and the complexity of your case.
What Is The Timeline From Start To Finish?
The complete treatment process unfolds over several months. Your initial consultation involves comprehensive imaging, including CT scans, and a thorough examination to develop your treatment plan.
The surgical placement appointment typically occurs within a few weeks of your consultation.
After implant placement, you wear your temporary prosthesis for approximately three to six months while the implants integrate with your jawbone through a process called osseointegration.
This healing period allows the implants to become fully stable and capable of supporting your final restoration.
Once healing is complete, we fabricate your permanent prosthesis, which involves taking detailed impressions and carefully matching the appearance to your preferences and facial features.
The final prosthesis offers superior aesthetics and durability compared to the temporary version.
How Do I Clean And Care For My All-on-4 Prosthesis?
Maintaining your All-on-4 restoration requires a specific care routine. Brush your prosthesis at least twice daily using a soft-bristled toothbrush, paying special attention to where the prosthesis meets your gum tissue.
Water flossers work exceptionally well for cleaning around the implants and underneath the prosthesis. We recommend specialized interdental brushes that can access the space between your prosthesis and gums.
Regular professional cleanings every three to six months allow us to thoroughly clean areas you cannot reach at home and monitor the health of your tooth implants.
Avoid using abrasive toothpastes that could scratch your prosthesis, as scratches can harbor bacteria. Your prosthesis is designed to be durable, but avoid chewing extremely hard items like ice or unpopped popcorn kernels.
Will My All-on-4 Prosthesis Look Natural?
Achieving natural-looking results is a primary benefit and focus of treatment.
As prosthodontists specializing in cosmetic and restorative dentistry, we design your final prosthesis to complement your facial structure, skin tone, and personal preferences.
We consider factors such as tooth shape, size, color, and the amount of gum tissue visible when you smile.
During the design phase, you have input on the appearance of your new teeth. We can create a smile that looks like a younger version of your natural teeth or design something entirely different if you prefer.
The prosthesis also supports your facial muscles, which can restore a more youthful appearance if tooth loss has caused facial sagging.
How Long Do All-on-4 Implants Last?
With proper care and maintenance, the implants themselves can last for decades or even a lifetime. The prosthesis typically lasts 10 to 15 years before requiring replacement due to normal wear or aesthetic updates.
Regular check-ups allow us to identify and address any issues before they become significant problems.
Factors that contribute to longevity include maintaining excellent oral hygiene, attending regular professional cleanings, avoiding tobacco use, and managing any medical conditions that affect bone health.
Can I Eat Normally With All-on-4 Implants?
Once you adjust to your new teeth, you can eat most foods you enjoy. The fixed nature of the prosthesis provides significantly more stability and biting force than removable dentures.
During the initial healing period with your temporary prosthesis, we recommend starting with softer foods and gradually introducing firmer textures.
After receiving your final prosthesis, most patients can eat steak, corn on the cob, apples, and other foods that were difficult or impossible with dentures or failing teeth.
The improved chewing ability often leads to better nutrition and greater enjoyment of meals.

At Prosthodontics of Princeton, we understand that considering a full-arch restoration is a significant decision.
